The demand for therapeutic donor insemination has been steadily increasing over the past few years. In most clinical applications, the procurement of cryopreserved semen specimens obtained from anonymous donors is the method of choice. Many recipients desire a more personal connection with their donor, to continue the genetic lineage of their partner by choosing a member of this family, to use a donor of a certain ethnicity which is otherwise unavailable through anonymous donor selection, or to provide their children the opportunity to have a relationship with the donor. ReproMed can assist the couple in Designated Donor cases.

Screening of the designated donor applicant, semen analysis/laboratory work and a 6-months quarantine of semen specimens is mandatory. Governmental agencies and professional organizations recommend the testing of designated donors and a 6-month quarantine of semen specimens. In equivocal situations, ReproMed will employ published standards, regulations and existing legal statutes.

The following procedure will be used in a designated donor situation.

1. Upon request, the designated donor applicant will be sent the Donor Application form. This must be completed by the donor applicant and returned to ReproMed prior to or at the time of first appointment.
2. The Donor Applicant form will be reviewed by ReproMed staff at the first appointment. ReproMed will establish a positive identification and assign a code number to the applicant. The applicant will need to provide the name, address, and phone number of the intended recipient as well as the referring physician. A semen specimen will be donated on the premises of ReproMed. A semen analysis will be completed and cryosurvival will be assessed. The semen analysis, along with the results of the quality control test freeze/thaw will be sent to the physician for approval and acceptance for future specimens. The Recipient Agreement will also be enclosed for the recipient and physicians signatures. The designated donor applicant fee is payable at the time of the appointment.
3. Upon ReproMed's receipt of the completed Recipient Agreement, the designated donor may schedule his next appointment. At that time, appropriate agreements and release forms will be signed. A blood specimen will be drawn and screened for infectious diseases such as: HIV-1/2, HTLV 1/2, HBsAg, Anti-HBc, HCV and VDRL. A semen specimen will be also screened for the presence of sexually transmitted diseases.
4. Satisfactory results in the previous steps will indicate that the donor into the DD program. Semen specimens will be collected and frozen. It is recommended that the designated donor provide a minimum of 4 specimens. The frozen specimens will be maintained in a 6-months quarantine after which the designated donor will return for the final screening. Copies of the Laboratory Records will be sent to the physician following the completion of all laboratory work. Release of the post-quarantined specimens will be made directly to the recipient's physician. All shipping fees will be billed directly to the recipient.
